I saw this pic in Todays Photos and I have a question about the gun setup shown.

My only experience with a machine gun was with an M 191A4, which fed from the left hand side. So how do these twin setups work? Are M2's built as either left hand feed or right hand feed, or is there a modification that is made to get the gun to run "backwards"?

You can make the M2 caliber .50 feed from the left or right.

I found the caption. This picture of this sailor bothers me. He needs a haircut, His sleeves are rolled about 5 inches too low, and the thing that took the cake is he has no cover/kevlar/or body armor at all. He looks like a bag of ass.

Could it be a: Mk 95 Twin .50 cal Machine Gun Mount
Apparently the US navy were planning on buying quite a few of them as a force protection measure.

Not particularly familiar with US Navy weapon systems but came across it when looking for a F.I.S.T. Powerpoint that was on their.

I'm pretty sure that it's still Navy reg hair.
You don't wear covers at sea. [Unless you are a watchstander.]
His Kevlar and steel pot are probably 'nearby'.
Sleeve regs, ack. You're right.

His shirt looks a little 'bulky' to me. could be just a gust of wind buy maybe his Kevlars being worn under his shirt?

Q. Are there any drawbacks to using a twin .50 set up? (bearing in mind it's no so common)

No, I haven't crossed the equator yet, but I know some form of hazing will occur when I do.
