Yup, dirt in the barrel, deformed ammunition objects stuck in the barrel etc. Not very uncommon, atleast not with small arms.

No one dies in it , pals ... It's called "Rose" . It's just the cannon exploding because of foreign objects inside the shell flying path (like dirt inside the cannon) .

yeah there is a hatch at the end of the cannon...you know so the hot gasses and gun blast dont fry the crew, and that protects them from things like this too.

I read over on tank.net that this was caused by one of the crew leaving the cleaning 'rod' in the barrel before they commenced a live shoot.

i thought that only happened when a barrel was loaded with 2 shells.. they did this during ww2.. to stop thousands of guns and tanks falling into nazi hands.. (british and russians).. it causes a the shell to explode but not leave the tube..

Yeah, this happens to all manner of high pressure weapons. And as previously stated, is usually the result of FOD Foreign Object Damage.
Cleanng rods fall into that category.
In the old days of muzzle loading muskets, it was not uncommon for a soldier to send his ramrod down range in the heat of battle.

hehe i did that this last weekend with my .50 cal..... Just wasn't paying attention and put the weapon down for a few min, went back picked it up, loaded a precussion cap and BANG! I see my ramrod flyin downrange as fast as it could go. at least I hit the target with it  course everyone promptly died laughing...*Sigh* I felt like an idiot....
