30.12.2004 - A lack of communication over in Iraq.

Last week - a C-23 Sherpa flew into a US operated airfield in Iraq during the day and saw there was construction equipment on the runway. Yet there was no NOTAM (notice to airmen). A trench was being dug in the runway, and it was not marked. It's a long runway and they just landed beyond the construction. They filed a safety hazard report that was immediately forwarded to the higher headquarters and to the Air Force wing based here. Well, it seems the construction continued, and still not marked or NOTAMed or anything. A C-130 landed on the runway the night of the 29th and didn't see the construction. It wound up going through what is now a large pot-hole in the runway. A few pictures are attached. The C-130 was totaled. There were several injuries to the crew and the few passengers that were on board but luckily nobody was killed. Quite the set of failures somewhere in the system regarding this improper construction and no notifications about it.

Based on the tire tread marks, the plane made it into the hole ok, but it didn't make it out to well - see that piece of the plane in the hole?

From what I know, that hole was actually made by runway maintanence

The Belgian C-130 crashed on july 15 1996 in Eindhoven (Holland).
41 people were killed (4 belgian crewmembers and 37 members of the Dutch Military Music Corps :| )

Forget the runway being shoddy, it should have no damn square training pot holes in it. That and competent people should be in charge of it.

It looks like a MC-130H Combat Tallon II from the 15th SOS to me.

A damned ****ing expensive aircraft! :|

:( That's one very expensive airplane. It indeed seems to be a MC-130H and not a regular transport version.

The one responsible will have his ass kicked big time!

Well...as I said, that hole was made by the maintenence crew....someone just neglected to tell the pilots it was there...

It happened during the end of the year, makes me suspect it was one of those things that happens when you're procastinating due to festivities.
I also wonder if that base was frequently used, i.e. it could be that the runway wasn't used all that often or for that matter, not open for operations but the Spec ops guys come unannounced.

In the world of aviation there are tons of safety measures but when Murphy strikes, he makes helluva entrance.

Thank God no one died, otherwise it would've been a meaningless way to go.

As a former base ops wiener - I cannot believe that nobody issued a NOTAM for a 100 foot hole in the runway. And I guess there was no tower, or rapcon, or ATIS - things must be pretty loosey goosey - makes for hazardous flying.

Singapore Airlines flight SQ006, daily service from Singapore to Los Angeles via Taipei, crashed on takeoff from Taipei's Chiang Kai Shek International Airport at 23:18 local time. The weather in the area was poor, precipitated by a typhoon,with a temperature of 69°F/21°C, pressure 29.59in./1002 hPa, wind from 020° at 37 knots, gusting to 56 knots, and visibility of 400 meters in heavy rain. Subsequent investigation of the accident confirmed that the flight crew mistakenly attempted takeoff on Runway 05R (9029x150ft), instead of the planned Runway 05L (12008x200ft). NOTAMs showed that, at the time of the accident, Runway 05R was closed for repairs, and that numerous pieces of construction equipment were parked on the runway. Approaching 140 knots, the 747 impacted a bulldozer and other construction vehicles, breaking the fuselage into three parts and igniting a large post-crash fire.

78 passengers died out of 179

My guess is the runway was cratered by airstrikes early on in the war, and its most likely being resurfaced- hence the whole.

So...what have we learned?

1> Fix large holes QUICKLY
2> Make sure eeeeverybody knows that there's a wopping great hole in the runway.
3> If it does occur, get pictures and add them to the other accidents like the airplane cleaners who forgot to turn off the washing machine and the mech boys who over pressurized the bird they were working on (they blew the tail off).

To keep the pilots on there toes ;)

What about some kind of air traffic controller? I know they had no tower but not even a radio operator that could have mentioned about the 100 foot square missing in the runway when the plane asked permission to land :cantbeli:
