In the movie THE BOURNE IDENTITY the US embassy marines use a funky looking M16. Its your every-day A2, but just past the forward sight, it looks as if the barrel has been cut down and fitted with an AK-74 type flash surpressor.

Is this a common modification? Is it at all realistic or just a Hollywood prop f*ck up?

At the MSG School you train with the Colt 9mm Submachine Gun and thats what you probably saw. The gun looks very similar to the M16, but with a 9mm banana clip, shorten bbl, and collapsable stock. I saw the movie, but I can't remember the scene.

Yes, the movie had a few technical inaccuracys in that area. As I recall, one Marine was carrying a Israeli Galil, hardly standard issue, lol.

You mean a patriot? They used that in Tom Clancy's Clear and Present Danger which was William Dafoe's weapon when he went to rescue the captured soldiers. http://www.tokyo-model.com.hk/cus_e.html the first 2 of the guns are the patriot's there are lots of variants, these are just a few.
