I have broken bones etc but getting a filling without an injection was the worse pain.
When the drill vibrates off the side of the tooth.....
The 2nd filling i had was lower right side right at the back and i asked for the injection and it hurt more than the filling! Had to go 2 inches into my gum!
And the pain that followed was terrible.
I was 7 or 8 years old as I remember when I got hold of a long bullwhip and brought it to school. After some time of easy does it practice with it during recess, in front of a lot of other students, I reared back and gave the whip all I had aiming to hit a basketball on the grass in front of me. It was a powerful but clumsy try and my timing was way, way off. The tip came nowhere near the basketball. Instead, like a lightning bolt, the tip hit me solidly behind the right ear. I remember a crack of thunder, astounding white hot paralyzing pain, then nothing. The witnesses say I passed out instantly. Some kids thought I had killed myself and started screaming and crying. For over a month my ear looked like a purple catcher's mitt.

I had testicular cancer when i was 17, but I wouldnt consider my worst physical pain. Come to think of it, I never experienced a really painful injury (been in a car accident and broke a few bones, though), I must be a lucky dude.
Kidney stone. 2002, while living in South Korea.
Had some boiling water in a pot fall onto my bare skin on my leg, took months to heal.
I've also been hit by a car, but that didn't hurt at all (yay adrenaline).
Recurring kidney stones over the past 20 years. Had litotrypsy (ultrasound, which itself is not too painful) 6 times to break up the stones so the particles could pass. Cannot tolerate opiates so am really looking forward to the next stones to form.
2 broken ribs with one in my left lung. With broken ribs a 6 year old girl can give you a beat down.

Me too. I just bounced off into a ditch. Only a bruise from that.
Slamming 3 fingers in a truck door... hard (it wouldn't latch else). Lost two nails and some skin. No broken bones. Worst part was that it locked closed and I had to dig out the keys from my right jean pocket with my free left hand and unlock it to free myself.
All in all, I've been lucky.
9 years old: ALMOST managed a back flip off of a couch and landed on my arm. Broke my forearm bone and bicep arm bone in half. Passed out on the car ride over to the children's hospital.
18 years old: Got hit from behind playing Junior A hockey, dislocated shoulder. Instead of the arm bone going under and getting jammed outside of the socket (which is the most common way an arm dislocates) well, that little ****er decided to brake through my shoulder socket. Ripped muscle and shattered cartilage.
The night after the surgery was almost worse than the incident itself.
Broke my left arm in 1980 that hurt like crazy. My appendicitis was nothing compared to post surgery +2 that was in November 2010.
Broken hands, arms, toes. Worst pain i can recall was riding a horse into a stinging tree. Burnt for 24 hours. Anecdotal evidence suggests people have died from using the leaves as toilet paper.
